REHABILITATION SERVICESRehabilitation is a process that aims to help a person achieve the best possible level of independence following an illness or period of disability. Rehabilitation programmes are designed for individual people and may include support to help with building confidence, nursing and therapy, such as physiotherapy, speech and language therapy or occupational therapy. The programme may be carried out at home, in one of the rehabilitation centres or a combination of the two. The following can make referrals for assessment for rehabilitation: Health and social care professionals: GP, District Nurse, Social Worker, Physiotherapist, Occupational Therapist and Speech and Language Therapist. |
