Diabetes Services


DIABETES SERVICES

The Trust’s Diabetes service aims to provide ongoing information and

support for children and adults with Type 1 and Type 2 diabetes at clinics or in the patients home and in Residential and Nursing Homes.

Diabetes services are provided by all nursing disciplines supported by the Diabetes Specialist Nurse. Referrals are accepted from any healthcare professional as well as patients/clients and carers directly.

Patients with Diabetes are seen at their GP surgery or local Hospital

Clinic. Housebound patients are seen at home by the district nurse, GP or Diabetes Specialist Nurse.

Following initial diagnosis the Trust provides a one-stop clinic called

Diabetes and You (DAY). At this clinic newly diagnosed patients with

Type 2 diabetes receive a comprehensive assessment and education

carried out by the Diabetes Specialist Nurse, Dietician, Podiatrist and

Physiotherapist. Patients may be referred to the local leisure centre for exercise programmes. Group evening sessions are held monthly for further education and support. Referrals for the (DAY) Clinic are only accepted via the GP.
