CARE MANAGEMENT
Care Management is the process by which people who have complex needs are helped to decide how these needs can be met. A comprehensive assessment will be co-ordinated by a Health and Social Services Worker, with full involvement of the person and, if selected, their family/carer. A meeting will then take place to determine the most appropriate care for that person. The carers needs will also be taken into account as part of this process. This does not prevent the carer having an assessment in their own right. Please ask your Social Worker, District Nurse or GP if a referral needs to be made to Care Management.
