LEAVING CARE AND AFTERCARE SERVICESUnit 4 First Floor Legahory Centre Craigavon BT65 5BE Leaving and Aftercare Service is provided by a Social Work Team who are dedicated to working with Young People in the age range 16-21 years who;
Our Role:Is to help Young People moving towards Independence. We work alongside the Young Person and his parents or carers to plan a programme of support, which will develop skills and knowledge that he/she will need in adulthood. It is a fact that each Young Person's needs are particular to them and that support packages must be able to respond to each Young Person's individual circumstances. Young People's active and ongoing involvement in the identification of their own needs is central to the success of our work. Our plan is to build on Young People's strengths and to support existing networks in promoting the integration of Young People in the community. Young People are referred to the Service:Usually by a Social Worker from the various teams e.g. Child and Family Care, Family Placement, or Disability Service. If the Young Person is no longer "Looked After" and has no direct contact with a particular Social Worker the referral can be a "Self Referral".
