Transforming Your Care: One Person at a Time
20/08/2014The Southern Trust is hoping to welcome members of the public to a conference - Transforming Your Care: One Person at a Time on Thursday 02 October 2014 at 3.30pm in Seagoe Parish Centre, Portadown.
The conference is aimed at everyone within the Southern Trust area who would like to share, or hear how people’s experiences have been enhanced by the changing face of Healthcare in the area, particularly through Transforming Your Care.
Service users and carers will have the chance to showcase how Transforming Your Care has had a direct impact on them and the care they receive and to talk about the changes they have seen in the past number of years.
Trust Chair, Roberta Brownlee says:
“We recognise that although many services are already transformed, we are continuously striving to change our care and services for the better, with the patient/client experience always at the heart of what we do. This conference will be an opportunity for us to reflect upon how the changes we are continuously making are affecting the people in our local area and to find out what is working well and what could be done better.”
Chief Executive, Mairead McAlinden also emphasised:
“Delivering good care that meets the needs of people who use our services is at the heart of everything the Trust does, and what Transforming Your Care is all about. The vast majority of this care is delivered in people’s home and local communities and there has been a real change with new community teams, services and technology. This has changed our hospital service – we now focus on people who are acutely ill and need very expert care – and more follow up care is provided outside hospital. In all this change, we have tried to improve the experience of people who use our services, whether this is about more choice, more independence or more say in their care.
This conference will enable those people who have experienced these changes to share that experience with us, and that will help us understand what is working well and where we need to keep improving to ensure that patients and clients are getting the services they require from the right people, in the right place at the right time.”
